</div><div>As a Care Manager, the primary responsibilities are: Care Management and caregiver Oversight.<br><br>

</div><div>· Oversee and guide the care provided to clients by conducting regular home visits to ensure care plan is addressing needs of client<br><br>
</div><div>· Conduct new client assessments and develop service plans<br><br>
</div><div>· Review and update care plans by collaborating with Caregiving team<br><br>
</div><div>· Independently manage weekly visit schedule and documentation<br><br>
</div><div>· Develop and foster on-going relationships with referral sources and attend key marketing events<br><br>
</div><div>· Provide coaching, support and assistance to Caregiving team<br><br>
</div><div>· Attend weekly Care management meetings<br><br>
</div><div>· Assist with Caregiver orientation and training<br><br>
</div><div>· Participate in on call and holiday rotation<br><br>
</div><div>· Provide back up client care occasionally if an urgent situation occurs<br><br>
</div><div><strong><u>Requirements</u></strong></div><div><br></div><div>· Three or more years in home care or a nursing facility or combination of facility and home care<br><br>
</div><div>· Must have excellent communication skills with the ability to build relationships with caregiving staff, clients, families and referral sources<br><br>
</div><div>· Must be able and willing to travel throughout the day to client locations.<br><br>
</div><div>· Prior experience in supervising staff perfered<br><br>
</div><div>· Must be have basic computer skills and proficient in Microsoft Office Suite Products</div>
