Position Overview: Health Advocate
The Health Advocate within our Solid Results Group serves as a crucial link between arenaflex and our Medicare members. In this role, you will engage with individuals through multiple channels—primarily telephone and digital platforms—to educate, motivate, and support healthy behaviors through innovative, compassionate, and empathetic communications.
Your primary objective will be to ensure that every member has a clear understanding of the valuable benefits available to them under their Medicare plan, particularly regarding preventive screenings, medications, and other essential health services. By delivering exceptional support and building trusting relationships, you will help close gaps in care and improve Star health outcomes for the members we serve.
Key Responsibilities
As a Health Advocate at arenaflex, you will be responsible for a comprehensive range of duties designed to support our Medicare members:
- Member Outreach and Engagement: Conduct inbound and outbound calls to Medicare members who may benefit from assigned services, including healthcare visits, preventive screenings, vaccinations, and medication refills. Your empathetic approach will help members feel valued and understood.
- Barrier Identification and Resolution: Effectively capture barriers to care and collect essential data while using motivational interviewing techniques to provide solutions that facilitate closing gaps in care. You will deliver top-tier support to every member, ensuring their needs are addressed with professionalism and respect.
- Care Coordination: Serve as a resource for members and assist in coordinating care between health plans, physicians, and pharmacies. You will help navigate the complex healthcare system and ensure members receive the services they need.
- Clinical Documentation: Actively listen and collect member-reported data elements in the CRM tool to support clinical record retrieval for HEDIS clinical record documentation of services provided.
- Information and Education: Provide members with the right information at the right time to help them make better decisions about their health and healthcare, improving member experience, retention, and growth through efficient coordination of services.
- Personalized Support: Engage, counsel, and educate members based on their unique needs, preferences, and understanding of available services. Accept end-to-end accountability for the member experience and deliver exceptional support.
- Issue Resolution: Respond to questions and resolve issues as a single point of contact, escalating to leadership as appropriate. Take ownership of member-assigned issues and connect them with additional services when needed.
- Relationship Building: Build trusting relationships with members by taking responsibility to fully understand their needs. Walk members through programs, arenaflex tools, and resources that support healthcare behaviors.
- Documentation and Tracking: Report and track all member contacts, events, and outcomes through appropriate systems and processes, ensuring accurate records and follow-up.
- Professional Communication: Use professional interpersonal skills to build relationships with both internal and external members and constituents. Utilize system tools and resources to produce quality business communications, including letters and spreadsheets in response to inquiries.
- Multi-Functional Excellence: Handle multiple functions and products while maintaining and exceeding performance standards. Identify issues that require escalation and provide suggestions for resolution.
